Assisted living and memory care

How New Jersey licenses assisted living, and when memory care is a separate dementia-home license.

This is general information, not a coverage decision and not medical advice. Rules change, and a Medicare Advantage plan can differ from Original Medicare. Confirm a specific stay with Medicare, the plan, the hospital discharge planner, or your county Aging and Disability Resource Connection.

Assisted living and memory care are often advertised as a pair. The Department of Health licenses them separately. This directory follows the license, not the brochure.

Assisted living licenses

  • An Assisted Living Residence is apartment-style housing, meals, and assisted living services for four or more adults. Residents generally have a lockable apartment.
  • A Comprehensive Personal Care Home provides room, board, and assisted living services. The living space is usually a bedroom rather than a full apartment.
  • An Assisted Living Program brings assisted living services into certain subsidized housing. It is not a stand-alone assisted living building.

Memory care in this directory

A Residential Dementia Care Home is a smaller, secured setting licensed specifically for people living with dementia. Those homes are listed under memory care. An assisted living building may market a memory care wing. That wing is not given its own listing unless the state licensed it as a Residential Dementia Care Home. If the license says Assisted Living Residence, that is all this site claims.

What the public file leaves out

Monthly private-pay price, current openings, and whether the community accepts NJ FamilyCare or MLTSS are not in the license file. Ask the community.

For NJ FamilyCare / MLTSS, see the NJ Medicaid and MLTSS guide. This page does not explain eligibility or payment.

  • Is the door lockable, and is the unit an apartment or a bedroom?
  • How is a dementia household secured, and can people still go outside?
  • Who gives medications, and is that included?
  • What happens when care needs increase?
  • May I see the latest Department of Health inspection?
